If Canceling Your Appointment, Please Give a Notice of 2 Business Days to Avoid a $35 Cancellation Fee. There will Be a $35 Fee on All Returned Checks.

Co-Pays and Co-Insurance are Due at the Time of Service, Unless Other Arrangements Have Been Made.

Knowing Your Insurance Benefits is Your Responsibility. Please Be Aware That Any Remaining Balance Not Paid By Your Insurance is Your Responsibility.

Dr. Cole, or a covering physician, is available during the evenings and weekends for patients who have urgent medical needs. Please call 911 if you are experiencing a medical emergency.

Every patient is encouraged to have a primary medical physician to handle non-allergy/immunology related issues.
Prescription refills are handled during normal business hours. Please allow at least 24 hours for routine prescription refills.

Antibiotic Policy

The physicians of Park Lane Allergy and Asthma Center do not prescribe antibiotics after business hours or over the weekend. This policy is in place to ensure appropriate treatment of an illness and to prevent over-utilization of antibiotics. Please contact our office during normal business hours to schedule an appointment.

Anonymous Call Rejection

Allow the doctor to call you back

When expecting a return call from our physician, please be sure to set your phone to allow blocked calls. If this process is not complete, we will not be able to return your phone call. Instructions on how to do this can be found here.

Anonymous Call Rejection prevents calls from ringing into your home if the caller’s line is “blocked.”
